デキサメタゾンが、コロナ診療に話題になっています。

まず以下を知ってもらった上で、ニュースを読んで下さい。

・デキサメタゾンは、ステロイド薬であって、
抗ウイルス薬では無い。

・元々ステロイド薬は、感染症を悪化させる可能性がある。

The NEW ENGLAND JOURNAL of MEDICINEにその原著論文がありました。

The RECOVERY trial was designed to evaluate the effects of potential treatments in patients hospitalized with Covid-19 at 176 National Health Service organizations in the United Kingdom and was supported by the National Institute for Health Research Clinical Research Network.
A total of 2104 patients were assigned to receive dexamethasone and 4321 to receive usual care.
In this controlled, open-label trial comparing a range of possible treatments in patients who were hospitalized with Covid-19, we randomly assigned patients to receive oral or intravenous dexamethasone (at a dose of 6 mg once daily) for up to 10 days or to receive usual care alone. The primary outcome was 28-day mortality.
The trial is being coordinated by the Nuffield Department of Population Health at the University of Oxford, the trial sponsor.
In the dexamethasone group, the incidence of death was lower than that in the usual care group among patients receiving invasive mechanical ventilation (29.3% vs. 41.4%; rate ratio, 0.64; 95% CI, 0.51 to 0.81) and in those receiving oxygen without invasive mechanical ventilation (23.3% vs. 26.2%; rate ratio, 0.82; 95% CI, 0.72 to 0.94) (Figure 2B and 2C). However, there was no clear effect of dexamethasone among patients who were not receiving any respiratory support at randomization (17.8% vs. 14.0%; rate ratio, 1.19; 95% CI, 0.91 to 1.55)
The RECOVERY trial provides evidence that treatment with dexamethasone at a dose of 6 mg once daily for up to 10 days reduces 28-day mortality in patients with Covid-19 who are receiving respiratory support. We found no benefit (and the possibility of harm) among patients who did not require oxygen.

結果として、デキサメタゾンは、
人工呼吸器、人工呼吸器を使用しない酸素投与が必要な患者の死亡率を減少させたが、
酸素投与を必要としない患者には、明らかな効果は無かった。

以前から言われている通り、
コロナ感染によって、サイトカインストームが起きて、
その結果、ARDSになって死亡する訳で、
いかにサイトカインストームを起こさないことがコロナ治療の鍵です。
デキサメタゾンは昔からあるステロイド薬ですが、
その抗炎症作用でサイトカインストームを抑えることが目的であって、
コロナっぽいと思ったからと言って、
デキサメタゾンを何でもかんでも使えば良い訳では無いです。
